Cancelled Service: Returning Equipment/Confirmation of disconnect

I unfortunately had to call multiple times to get our service disconnected, got overcharged, promised a refund and never saw it.  All I am asking for at this point is to get a confirmation of our service being disconnected and a confirmation of the promise we do not have to return our equipment due to the age of it, but I have not received it after 3 calls/requests.  We want it in writing before being surprised by a bill for unreturned equipment, but no representatives have been able to complete their promise. Can you provide this?
