Return of Modem/Radio after termination of service

My exede service was terminated on August 14. I need to return the modem/radio, but I have not received the return box or information. Perhaps it was mailed to my former residence address. How do I contact customer service? I have the modem and cables, but the phone numbers for ViaSat are in storage.

Nancy, please call Customer Service at 855-463-9333, or send an email to the moderators at exedelistens@viasat.com. Include your account number and phone number associated with your old account, along with a brief description of your problem.

If you don't have your account number, you can include the MAC address located on your router on the ID tag. This number consist of 6 groups of 2 letters or numbers separated by a colon.
