Have not received box to return tria

I cancelled service on 9/14/16 @ 10pm with Jacquelin.  I have not yet received the box to return Tria, modem, etc.  What should I do.

that email address should work. You can also call 855-463-9333 and we can look up the UPS tracking number for that box
